Plantar Fasciitis: Why Your Heel Hurts and How Physical Therapy Can Help

Morning heel pain is common, treatable, and usually managed without surgery. The most effective rehabilitation plans combine symptom relief with a gradual return to strength and activity.
Does your heel hurt when you take your first few steps in the morning?
Maybe the pain eases once you start moving, only to return after a long day of standing, walking, exercising, or working on your feet. You may also notice discomfort when getting up after sitting for an extended period.
These are common signs of plantar fasciitis, one of the most frequent causes of pain along the bottom of the heel. It affects active and sedentary adults alike.
The good news is that most people do not need surgery. An individualized rehabilitation program can reduce pain, restore mobility, rebuild strength, and help you return to the activities that matter most to you.

What Is the Plantar Fascia?
The plantar fascia is a strong band of connective tissue that runs along the bottom of your foot, connecting your heel to the base of your toes.
It helps support the arch of your foot and manages force as you stand, walk, run, climb stairs, and push off the ground. Although the plantar fascia is strong, repeated stress can exceed its ability to recover.
When this happens, the tissue near its attachment to the heel can become painful and sensitive.
The word fasciitis suggests inflammation, but persistent plantar heel pain is often more complicated than inflammation alone. Research involving people with chronic plantar fasciitis has found changes in plantar fascia thickness and blood flow that are associated with pain and difficulty using the foot. It may therefore be more helpful to think of the condition as an overloaded and irritated tissue that needs gradually restored movement and load tolerance.
What Does Plantar Fasciitis Feel Like?
People with plantar fasciitis commonly describe:
Sharp, aching, or burning pain under the heel
Pain toward the inner portion of the heel
Significant discomfort during the first steps in the morning
Pain when standing after sitting for a long time
Symptoms that temporarily improve after moving around
Pain that returns after prolonged standing, walking, running, or exercise
Tenderness when pressing near the plantar fascia’s attachment to the heel
A physical therapist can often recognize plantar fasciitis through your symptom history and a physical examination. The examination may include checking the location of your pain, ankle and toe mobility, calf flexibility, foot and ankle strength, balance, walking pattern, and tolerance for standing or pushing through the foot.
Imaging is not always necessary when the symptoms and examination clearly fit plantar fasciitis. It may be considered when the diagnosis is uncertain, symptoms followed a traumatic injury, or another condition needs to be ruled out.
Why Does Plantar Fasciitis Develop?
Plantar fasciitis usually does not result from one single problem. It often develops when the demands being placed on the foot increase faster than the plantar fascia and surrounding muscles can adapt.
Common contributors may include:
A sudden increase in walking, running, hiking, or sports
Starting a new exercise program too aggressively
Spending more time standing or working on hard surfaces
Limited ankle mobility or calf flexibility
Weakness of the calf, ankle, foot, or toe muscles
A sudden change in footwear
Returning to activity after a period of inactivity
Repeatedly performing activities that load the foot without adequate recovery
This does not mean that you have a “bad” foot or that one particular foot shape is destined to cause pain. It usually means that the current demands on the foot are greater than the foot’s present capacity.
The goal of rehabilitation is not simply to avoid using your foot. It is to calm the irritated area while gradually improving your ability to tolerate the activities you need and enjoy.
Weakness Can Be Part of the Problem
People often focus only on stretching when dealing with plantar fasciitis. Stretching can be helpful, but strength matters too.
A JOSPT study found that people with plantar heel pain performed worse on tests of ankle plantar-flexor and toe-flexor strength than similar adults without heel pain. These muscles help support the foot, manage impact, maintain the arch, and propel the body forward during walking and running.
The study also found an association between longer self-reported orthotic use and lower muscle performance. This does not prove that inserts weaken the foot. However, it reinforces an important point: supportive devices should not automatically replace a progressive strengthening program.
What Treatments Does the Evidence Support
The updated JOSPT clinical practice guideline supports an individualized combination of treatments rather than relying on one passive treatment or quick fix.
1. Education and Activity Modification
Complete rest is rarely the long-term solution. Instead, your physical therapist can help you temporarily adjust activities that are aggravating the heel while maintaining as much comfortable movement as possible.
The goal is to reduce repeated irritation without allowing the foot and leg to become weaker or less tolerant of activity. As symptoms improve, activity is gradually rebuilt.
Walking or running distance
Exercise frequency
Hills, speed, or impact activities
Time spent standing
Work or household tasks
Footwear used during demanding activities
2. Plantar Fascia and Calf Stretching
The clinical practice guideline strongly recommends both plantar fascia-specific stretching and stretching of the gastrocnemius and soleus muscles of the calf. These techniques can improve pain and function over both the short and long term.
A plantar fascia stretch commonly involves gently pulling the toes upward until a stretch is felt through the arch. Calf stretches may be performed with the knee straight and bent to target different portions of the calf.
The correct position, intensity, and frequency depend on your symptoms. More force is not always better, especially when the heel is highly sensitive.
3. Progressive Foot, Ankle, and Calf Strengthening
Current guidelines recommend resistance exercise for the muscles of the foot and ankle.
Strengthening exercises should be challenging enough to promote adaptation without repeatedly causing a major flare-up.
Some discomfort during rehabilitation may be acceptable, but pain should be monitored. Your response later that day and the following morning can provide valuable information about whether the exercise level was appropriate.
Controlled calf raises
Seated or standing heel raises
Toe-flexor exercises
Arch-control exercises
Balance activities
Single-leg strengthening
Gradual walking, running, or jumping preparation
4. Manual Therapy
The guideline recommends manual therapy directed toward relevant joints and soft tissues of the lower leg and foot. This may help address mobility restrictions, improve movement, and reduce pain when combined with exercise and other active treatments.
Manual therapy might include treatment of the ankle, foot, calf, or surrounding soft tissues based on what is found during your evaluation.
Hands-on care can be useful, but it should generally support—not replace—strengthening, stretching, education, and progressive activity.
5. Taping
Rigid or elastic taping techniques may provide short-term pain relief and improve function, particularly during the first several weeks of care. The guideline recommends using taping alongside other physical therapy treatments rather than as the entire treatment plan.
Taping can also help determine whether temporary arch support changes your symptoms before considering an insert.
6. Shoe Inserts and Orthotics
Prefabricated or custom orthotics may help some people when combined with stretching, strengthening, manual therapy, and activity modification.
However, the guideline does not recommend using an orthotic as an isolated treatment for short-term pain relief. An insert may reduce symptoms or redistribute pressure, but it does not automatically restore ankle mobility, muscle strength, balance, or activity tolerance.
The best device is not necessarily the most expensive one. Whether an insert is appropriate should depend on your symptoms, goals, footwear, examination findings, and response to a short trial.
7. Night Splints
A night splint holds the ankle and toes in a gently lengthened position while you sleep.
The guideline recommends a one- to three-month night-splint program for people who consistently experience significant pain with their first steps in the morning.
Night splints are not necessary for everyone. Comfort, sleep quality, symptom pattern, and tolerance should all be considered.
8. Other Treatment Options
Dry needling or low-level laser therapy may be considered for selected patients as part of a broader plan of care. Their usefulness depends on the individual, clinician training, treatment goals, and response to more established interventions.
Therapeutic ultrasound should not be routinely added simply to enhance the benefits of stretching. The guideline favors manual therapy, stretching, exercise, and appropriately selected orthoses over relying on electrotherapeutic modalities for longer-term improvement.
Do You Have to Stop Walking or Exercising?
Not necessarily.
Some people need a temporary reduction in painful activities, particularly running, jumping, or prolonged standing. However, completely avoiding movement for an extended period can reduce the strength and capacity needed for recovery.
A better approach is often to find a manageable activity level and then rebuild gradually.
For example, a runner may temporarily reduce distance, speed, hills, or consecutive running days rather than stopping all exercise. Someone who stands at work may benefit from scheduled sitting breaks, changes in footwear, taping, or adjustments in how long they remain in one position.
Your plan should be based on how your foot responds—not on a universal rule that everyone must rest for a certain number of weeks.
How Long Does Recovery Take?
Recovery time varies. Your timeline may depend on:
How long the pain has been present
The irritability of the heel
Your work and activity demands
Foot and ankle strength
Ankle mobility
General health
Consistency with your home program
Whether activity can be temporarily modified
Whether another condition is contributing to the pain
The 2025 JOSPT payer perspective emphasizes the importance of setting reasonable expectations for the duration of physical therapy, number of visits, and expected clinical outcomes rather than treating plantar fasciitis as a one-visit problem.
Progress is not always perfectly linear. A more active day may temporarily increase symptoms without meaning that the tissue has been damaged again.
Your physical therapist can help you interpret these changes and adjust the program without repeatedly starting over.
When Heel Pain May Be Something Else
Typical plantar fasciitis usually develops gradually. Seek an evaluation sooner if your pain began with a specific injury or is accompanied by:
A sudden pop or tearing sensation
Immediate severe pain
Significant swelling or bruising
Difficulty bearing weight
Persistent numbness or tingling
Pain that wakes you at night and is not affected by position
Fever, unexplained illness, or an open wound
Rapidly worsening symptoms
A JOSPT case report described a competitive soccer player whose mechanism of injury and examination raised concern for a plantar fascia tear. MRI later confirmed a high-grade partial tear with localized swelling. An acute tear requires a different evaluation and management approach than typical gradual-onset plantar fasciitis.
Other sources of heel pain can include nerve irritation, a bone stress injury, heel-pad irritation, or pain referred from another region. A thorough examination helps determine whether your symptoms match plantar fasciitis or require additional medical evaluation.
How Physical Therapy Can Help
Physical therapy is more than receiving a few stretches or having the painful area massaged. A comprehensive evaluation can help determine:
Why your foot may be overloaded
Which movements are limited
Which muscles need greater strength or endurance
Whether taping or an insert provides useful support
How your footwear affects symptoms
Which activities should be modified temporarily
How to safely return to walking, running, sports, work, or exercise
Research examining more than 800,000 people diagnosed with plantar fasciitis found that only a small percentage received a physical therapy evaluation. However, most of those who did receive physical therapy were provided supervised rehabilitation, frequently combined with manual therapy and other guideline-supported care.
The most effective program is not a generic list of exercises. It is a progressive plan matched to your current symptoms, daily demands, and personal goals.
The Bottom Line
Plantar fasciitis can be frustrating, especially when every morning begins with a painful first step. But heel pain does not necessarily mean that you need to stop moving or live with the problem indefinitely.
Current evidence supports a combination of:
Education and temporary activity modification
Plantar fascia and calf stretching
Progressive foot, ankle, and calf strengthening
Manual therapy when appropriate
Short-term taping
Selective use of orthotics
Night splints for persistent morning pain
Gradual return to meaningful activity
Passive treatments may help reduce symptoms, but long-term recovery usually depends on rebuilding the foot’s capacity to handle everyday life.
